Specify the trigger, destination, required brief, caller message, and fallback for each escalation route. Name the team responsible after transfer.

For each conversation, mark every criterion met, unmet, or not applicable, and attach the relevant transcript or event evidence.

Fix the weakest transition
Track successful connections per attempted transfer, context completeness, ownership gaps, and repeat explanations. Separate unavailable destinations from incorrect routing so the right owner can act.

What evidence should reviewers use?
Use transfer events, the brief the recipient actually receives, and the conversation before and after handoff. Record evidence for each judgment rather than scoring from impression alone.
